In Google’s AI answers about behavioral health EHRs on 28 October 2026, 490 of 666 links (74%) led to vendors’ own pages, including the “best EHR” lists vendors publish themselves. Which EHRs a buyer sees changes with the question they ask.

A therapist opening a private practice, a psychiatric nurse practitioner who needs e-prescribing and an addiction treatment center with residential beds all need an EHR. When they search Google, the first thing they often see is an AI answer.

Google shows AI answers in two places: AI Overviews, a summary with links inside the regular search results, and AI Mode, a separate AI search where you can ask follow-up questions. We asked both the same 20 questions in buyers’ words, from “best behavioral health ehr” to “therapynotes alternatives,” three times each, as a user in the United States, and saved every answer with the EHRs it named and the pages it linked to. AI Mode answered all 60 times. An AI Overview appeared for 54 of the 60 searches, so the study has 114 answers.

A name in an answer is not always a suggestion. In answers about community mental health centers, SimplePractice appears only as the kind of tool solo practices use. In “simplepractice alternatives” it is the product being replaced. The matrix and the tables below leave such mentions out.

What Google’s AI said about the most-named EHRs

On 28 October 2026, these were the eight EHRs Google’s AI named most often. For each: the kinds of practice it was named for, one of the lines about it that came back word for word most, how often answers linked to its own site and which page its entries linked to most. The lines are Google’s words, not Wonderflow’s, and we have not checked them against the products. In the entries quoted here, lines about SimplePractice, TherapyNotes and Sessions Health linked to lists published by RXNT and DeepCura, two companies that sell software to practices themselves.

SimplePractice: named in 80 of 114 answers

In the matrix below, SimplePractice is named for six of the eight kinds of practice, all but addiction and residential treatment and community mental health centers. Which EHRs Google’s AI names for each kind of practice

On 28 October 2026, none of the 15 EHRs Google’s AI named for private practice came up for addiction and residential treatment. We grouped the 20 questions into eight kinds of practice and need. A filled cell means Google’s AI named the EHR in at least two of three runs for at least one question of that kind, in AI Mode or in AI Overviews. There is no total per EHR: filling more columns doesn’t rank an EHR higher.

SimplePractice vs TherapyNotes in Google’s AI answers

On 28 October 2026, Google’s AI tied SimplePractice to a client portal, telehealth and solo practitioners, and TherapyNotes to billing and insurance. Of the 114 answers, 51 suggested both, and SimplePractice was the first EHR named in 31 of them and TherapyNotes in 7. The count leaves aside the questions about replacing either, the answers about community mental health centers and mentions for contrast.

Table 10 Words Google’s AI used in its list entries on each EHR: share of 62 entries on SimplePractice and 58 on TherapyNotes, 28 October 2026.

“user-friendly”14/622/58
“billing”21/6244/58
“insurance”16/6242/58

An entry is the first list item or table row of an answer with the EHR’s name in its first line. One entry per answer. Words are counted in the entry’s text, not in link addresses. Left out: answers to the question about replacing the product itself, and one item that names both only as tools for another kind of practice.

Source: Wonderflow record of Google AI Mode and AI Overviews answers, 28 October 2026.

The answers framed the two differently. Entries on SimplePractice mentioned a client portal in 44 of 62, telehealth in 38 and solo practitioners in 36. Entries on TherapyNotes mentioned billing in 44 of 58 and insurance in 42. These are the answers’ words, not our comparison of the products.

Some labels stuck. Each line below came back word for word in at least two distinct answers, not only in repeated runs of one answer.

Which pages the answers cite

On 28 October 2026, the 114 answers from Google’s AI Mode and AI Overviews carried 666 links. Of these, 490 (74%) led to the sites of companies that sell software or services to practices: product pages, blog posts and their own lists of the best EHRs. All but one of the 114 answers link to at least one such page.

The most-cited list in this study comes from a company the answers never named. DeepCura’s “Best EHR for Private Practice (2026): 10 Systems Ranked” was cited in 24 of the 114 answers, in 21 different answer texts, and none of the 114 named DeepCura. It was cited in 19 of the 74 answers that named TherapyNotes and in 17 of the 42 that named Sessions Health. Only OmniMD’s page about its own mental health EHR was cited as often, in 24 answers with 12 different texts.

Ease Health and ClinicMind show the same pattern on a smaller scale. Pages on their sites were cited in 18 and 13 answers, while each company was named in 5. In the AI Mode answers to “best behavioral health ehr” and “best mental health ehr software,” the name Qualifacts was itself a link to an article on Behave Health’s blog. That happened in six answers with two different texts.

If you’re choosing a behavioral health EHR

If you use Google’s AI answers to start a shortlist, four things in the answers we saved on 28 October 2026 are worth knowing. They describe what Google’s AI said that day, not which software fits your practice.

  1. Most links lead to companies that sell to practices

Of the 666 links, 490 led to the sites of companies that sell software or services to practices, EHR vendors among them. DeepCura’s list of the best EHRs for private practice was cited in 19 of the 74 answers that named TherapyNotes.

  2. AI Mode and AI Overviews give different lists

On each of the 19 questions both AI Mode and AI Overviews answered, the lists differed. For “best ehr for community mental health centers,” they had no name in common.

  3. The most-named EHRs are not named for every kind of practice

SimplePractice and TherapyNotes did not come up for addiction and residential treatment or for community mental health centers.

  4. Asking again is not a second opinion

In 10 of the 39 groups of three runs, all three returned the same text word for word.

If we were building a shortlist from these answers, we’d check three things first. Start from your kind of practice, not the overall count. Open the page behind any “best for” label: it can be another vendor’s list, so read it as that company’s view. And ask your own question in both AI Mode and AI Overviews, since they gave different lists for every question both answered.

If you sell a behavioral health EHR

Three things in Google’s AI answers of 28 October 2026 matter if your company sells an EHR. They describe what the answers said that day, not how Google decides.

  1. Being named and having your own page cited are different

SimplePractice was named in 80 answers and its own site was linked in 14. Behave Health was named in 25 and its site linked in 24.

  2. Other companies’ lists describe you

DeepCura’s list of the best EHRs for private practice was the link in TherapyNotes’ entry in 13 answers and in SimplePractice’s in 11.

  3. AI Mode linked to vendors more often than AI Overviews

On 28 October 2026, 250 of 314 links (80%) in AI Mode led to vendors, against 240 of 352 (68%) in AI Overviews.

Missing from the answers, or described wrong?

The tables above show every EHR Google’s AI named in at least two of three runs. In all, the answers named 70 companies, some only once, and linked to the sites of 24 more that they never named. Download the list of all 94 companies (CSV) to see how often each was named and how often answers linked to its site.

If this page misstates what an answer said, write to hello@wonderflow.agency with the link. We reply within five business days. We check what you sent against the saved answer and correct the page if we got it wrong. Corrections are free and don’t depend on working with us.

We can’t change what Google says. The answers link to pages like the ones above: vendors’ own pages and other sites that describe them. When we measure again, the new results will appear on this page, and this measurement will stay available with its date.

Questions about AI answers on behavioral health EHRs

What is the best behavioral health EHR, according to Google’s AI?

In 114 answers from Google’s AI Mode and AI Overviews on 28 October 2026, SimplePractice was named most often, in 80 answers, followed by TherapyNotes in 74, Sessions Health in 42 and ICANotes in 41. The list changes with the question, and answers about addiction treatment named other systems. For the exact search “best behavioral health ehr,” AI Overviews named TherapyNotes, Sessions Health, ICANotes, Valant, Qualifacts, OmniMD, Epic, Oracle Health (Cerner) and Veradigm (Allscripts) in all three runs, which returned the same text, and did not name SimplePractice. AI Mode named SimplePractice, TherapyNotes and 10 others in all three runs. This is a record of AI answers, not our recommendation.

Does Google’s AI name SimplePractice or TherapyNotes more often?

SimplePractice, by a small margin: 80 answers against 74 on 28 October 2026. Leaving out answers where either is the product being replaced or a contrast, the count is 66 against 63. Of the 51 answers that suggested both, outside the questions about replacing either, SimplePractice came first in 31. The answers tied SimplePractice to a client portal and telehealth more often, and TherapyNotes to billing and insurance.

What does Google’s AI say about TherapyNotes?

On 28 October 2026, entries on TherapyNotes mentioned billing in 44 of 58 and insurance in 42. One of the lines about it repeated most often was “Best for Insurance-Based Practices,” in six answers with four different texts. For SimplePractice, one of them was “Polished client portal and modern design.” These are the answers’ words, not our assessment.

Which pages do Google’s AI answers about EHRs link to?

Mostly to vendors’ own sites. Of 666 links in 114 Google AI Mode and AI Overviews answers on 28 October 2026, 490 (74%) led to companies that sell software or services to practices, including their own lists of the best EHRs. In AI Overviews alone the share was 240 of 352 links (68%), in AI Mode 250 of 314 (80%). The rest led to video, forum and social sites such as YouTube and Reddit (94), publishers, review sites and directories (59), other sites (15) and general-purpose software companies (8).

How do EHRs get into Google’s AI answers?

We can’t see how Google decides what goes into AI Overviews or AI Mode. We can see what the answers link to. In our study of 114 answers on 28 October 2026, 490 of 666 links (74%) led to vendors’ own sites, including their lists of competitors, and a company could be cited without being named: DeepCura’s list was cited in 24 answers and DeepCura in none.

Is this a ranking of EHRs?

No. It records which EHRs Google’s AI named on 28 October 2026 and which pages it linked to. We have not tested or scored the software.

Is any company named here a Wonderflow client?

No. None of the companies named on this page is a Wonderflow client. Wonderflow sells search and AI visibility work to software companies and may offer it to companies named here. That does not change what this page reports.

Method and data

Research question
Which behavioral health EHRs does Google’s AI name when buyers ask for the best one, and which pages does it cite?
Searches
Twenty searches we wrote in buyers’ words, grouped into eight kinds of practice and need. All 20 are in the tables of section 03.
Where we asked
Google AI Mode and Google AI Overviews, United States, English, collected through the DataForSEO API.
Runs
Three runs of every question on each surface on 28 October 2026, 120 requests sent between 16:06 and 16:10 UTC. AI Mode answered all 60. AI Overviews appeared in 54 of 60.
Named
An EHR counts as named if its name appears in the answer text. Names were matched against a list of 70 companies and 85 spellings of their names, taken from bold text and links in the answers. A name counts as stable if it appears in at least two of three runs.
Cited
Every link in the answer text and in its list of sources, counted once per answer. A link counts toward a company if it leads to the company’s own site, including its blog and help pages.
Words
In section 04, an entry is the first list item or table row of an answer with the EHR’s name in its first line, one per answer. A word counts if it appears in the text of that entry, not counting link addresses.
What a figure means
How often Google’s AI named or cited a company on one day. Not the quality of the software, its fit for a practice or its market share.

Limits
  • ChatGPT, Perplexity and Gemini were not measured. This page covers Google’s AI only.
  • In 10 of 39 groups of runs (one question on one surface), all runs returned the same text word for word, so “three of three” there is one observation repeated. AI Mode also returned the same text for two different questions, “best behavioral health ehr” (two runs) and “best mental health ehr software” (three runs). Counting identical texts once, the 114 answers hold 93 different texts.
  • All 120 requests were sent within four minutes, so the three runs of a question are a minute or two apart.
  • AI Overviews did not appear for “best ehr for addiction treatment centers” in any run, and appeared in two of three runs for three other questions. The saved text of the three AI Mode answers to that question stops after introductory sentences that end with a colon. If a list followed, it was not captured. These sentences name ICANotes, Lightning Step, Behave Health and others only as sources, so we left them out of the matrix and the tables.
  • Named includes mentions for contrast. We left out of the matrix and the tables the contrast mentions we found when reading the answers: SimplePractice and TherapyNotes in the questions about their alternatives and about free EHRs, SimplePractice in answers about community mental health centers, seven mentions in single runs where an answer named SimplePractice, TherapyNotes or TheraNest only as tools for another kind of practice, and Headway and Alma, which AI Overviews named for psychiatrists as credentialing and scheduling services to pair with an EHR.
  • One day and one location. The data provider did not record the device type. AI answers change over time and from user to user.
